Our Refer-a-Friend program gift cards has no cash value, and it is not valid with any other special offer or promotion. Your referral should not be currently a patient of record. To be eligible to collect your prize, the referred patient must be qualified, which means they must come in to the office and have an exam, x-rays and a cleaning completed. Members of same household count as one referral.
